To expedite your first appointment, we have some forms for you to fill out. The first two pages can be completed on your computer and then signed after printing (adult form only, child form must be printed then filled out). Page three is acknowledgement of our office policies and requires a signature. Page four is acknowledgment of receipt of our privacy practices which are detailed in our Privacy Policy (pdf).
